Specialty highlights

A pharmacy partner for the moments that matter

We understand that transplant care doesn’t stop after surgery. Our pharmacists become trusted allies for patients, helping them navigate the ups and downs of immunosuppression, medication adjustments, and recovery. We focus on preventing complications, reducing hospital readmissions, and building strong, lasting partnerships with care teams.

Everyone deserves access to the medication they need

Navigating the cost of medication can be overwhelming. That’s why our dedicated Polaris Specialty Rx Patient Assistance Advocates are here to help. They work closely with patients to explore available financial support from drug manufacturers or independent foundations and guiding them every step of the way.

  • Verification of Assistance Options: Our advocates review each patient’s unique situation to determine if they may qualify for financial assistance from drug manufacturers or independent foundations.
  • Personalized Outreach: If assistance is available, a Patient Assistance Advocate will contact the patient to explain the options and offer step-by-step guidance through the application process.
  • Eligibility Review: Each program has its own eligibility requirements, typically based on household size, income level, and insurance status.
  • Prefilled Application Support: If the patient is interested, our team will provide a prefilled application by email or mail and request the patient return it with supporting documents such as proof of income and insurance cards.
  • Submission and Follow-Up: Once all required materials are received, Polaris submits the application and supporting documents to the manufacturer or foundation. We proactively follow up and keep the patient informed throughout the approval process.
  • Approval and Coordination: we notify the patient to coordinate medication delivery directly.
